Student-Athlete Athletic Training Forms
Laker Student-Athletes:
We are excited about the upcoming academic and athletic season.
We are confident the 2012-13 year is going to be the most rewarding
of your student-athletic career.
Below is a link to a list of some very important forms that need to
be read, signed, and returned, so that your participation can begin
immediately upon your arrival this upcoming academic and athletic
season. A personal health insurance plan, either an individual plan
or family plan that covers intercollegiate athletic participation
is required for all student-athletes for the 2012-13 year. For
information regarding approved insurance carriers and other
insurance options, please contact Athletic Trainer John Zubal at
(678) 466-4685.
The minimal coverage required must include:
- Coverage for accidental athletic injuries, full coverage not required.
- Have a maximum deductible of $1,000.00.
- Have a minimum coverage of $90,000.00.
Clayton State will continue to carry health insurance that will
provide secondary coverage, which will take care of your deductible
and expenses over $90,000.00. This will guarantee that you will
have no cost, other than your policy, for injuries incurred as a
result of your athletic participation. Please be aware that no one
will be allowed to participate in conditioning, practices or games,
until a health insurance plan is on file with the Clayton State
Athletic Department.

2. State of Georgia Waiver
3. Sports Physical Form (For Returning Students and New
Students)
4. Medical History Form
5. Substance Abuse/Drug Testing Form
6. NCAA Supplement Report
7. Weight Room Policy and Procedures

Please return all of the above forms to the listed address or
just give them to John Zubal upon arrival. No one will be allowed
to participate in conditioning, practices or games until
all necessary information is on file with the Department
of Athletics.
